GFX Request I need some channel art

I need some channel art for my YouTube.

What I need in the channel art:
-My channel name (SAMOISH Gaming)
-A subtext that says "Subscribe to join the SAMOISH Nation today!"
-My social media accounts (Facebook, Twitter, & Instagram)
-A Black Ops 3 Design (I don't mind what it is as long as it's from the game)
-Red, Black, and White colors (predominately Red & Black)
