GFX Request In need of a Minecraft channel banner

For my YouTube channel, I am in need of a Minecraft banner.
What I'd like to have:

• "LW001" in a big font size, font type the "TERMINATOR" font, as I use that in every one of my videos.
• My Minecraft skin (IGN LW001) on the side of it, holding a diamond sword if that is possible.
• Background Orange-yellow, maybe a bit of green.
